How to Make a Sports Insurance Claim
Step 1 — Immediately after the injury

Seek medical attention right away (GP/A&E) and follow the doctor’s advice. The insurer requires medical evidence.

Record facts while fresh: date & time, exact location, what happened, witnesses (names & contact), photos of injuries/scene and any match/fixture paperwork.

Tell your club (compulsory committee members, welfare officer if you have one & coach) and Student Activities so they can confirm you were a signed-up member and the activity details if the insurer requests club/Union confirmation.
Step 2 — Read the claim form & policy summary

Open the Chubb Personal Accident claim form here. It contains the fields the insurer requires and instructions about hospital/doctor sections.

Read the Insurance Product Information Document (IPID) here for what is and isn’t covered and any waiting periods (e.g., temporary total disablement waiting periods).
Step 3 — Complete the claim form correctly
Follow the form’s guidance carefully (all items below appear on the claim form):

Use BLOCK CAPITALS and black ink (if paper). Sign the declaration.

Complete claimant details (full name, DOB, full address — specify home or term-time).

Give exact date & time of the accident, where it happened, sport/activity, how it happened and injuries sustained.

Hospital Statement — only complete if claiming hospitalisation; must be completed by hospital staff (hospital staff/stamp, dates admitted/released).

Doctor’s Statement — the attending doctor must complete this; fees for completing it are the claimant’s responsibility.

Provide a password on the form (used by insurer to secure your claim info).
Step 4 — Gather supporting documents
Gather the below supporting documents to include with the form:

Proof of membership (receipt for Competitive/Participation membership and evidence you were a signed-up club member at the date of injury).

Completed claim form.

Doctor’s statement and/or Hospital statement (as required).

Receipts/evidence for medical treatment (GP, prescriptions), dental receipts (dental injuries cover), physiotherapy receipts (if applicable under the policy), travel receipts for emergency travel, hospital paperwork.

Proof of earnings (if claiming loss of earnings under the policy).

Photos, witness statements, club attendance/fixture lists or coach confirmation if available.
Step 5 — Submit the claim
Important: do not send the claim to the Student Activities team, as the insurer handles claims directly.
Step 6 — After submission: what to expect

The insurer will correspond directly with you about the claim, so keep an eye on the email/password that you provided.

The insurer may request further evidence or a medical examination (they will pay for insurer-requested medical exams).
