how to report an Accident or Near Miss

 

1) First/emergency actions

  • If someone is seriously injured or life-threatened, call emergency services (999) first, followed by University security (01895 255 786).

  • Prioritise safety: move people only if necessary, give first aid, contact on-site first aider.

  • Preserve the scene/evidence if safe to do so (don’t move damaged equipment unless needed).


2) Open reporting form

  1. Go to brunelstudents.com/accident.

  2. You’ll be redirected to Brunel University’s Health & Safety reporting page.

  3. Pick the correct form:

    • Accident Report - Use this if the incident caused injury, illness or accidental damage (e.g., injuries, gas leak, spill, loss of power, damaged equipment/infrastructure).

    • Near Miss Report - Use this if nothing was harmed but the event could have caused injury or damage (a close call, unsafe condition).


3) complete form

  • The form can be submitted by any student, but we recommend that one of the club/society's compulsory committee members (Chair, Secretary & Treasurer) submit it. 

  • Provide clear, factual answers - do not speculate. Use dates/times and names where known. Typical information to include:

    • Date, time & exact location of the incident.

    • Which club/society and whether it was an official club/society activity.

    • Names and contact details of people involved (and any witnesses).

    • Description of what happened - step-by-step, factual.

    • Injuries or damage sustained (describe body parts, severity, treatment given).

    • Immediate actions taken (first aid, ambulance called, equipment isolated).

    • Photos or documents (attach if the form allows).

    • Any unsafe conditions or equipment involved (make/model/serial if relevant).

    • Suggested corrective actions or anything you think will prevent recurrence.

  • When finished, click Place Request to submit this report.


4) After you submit

  • A ticket is created on the Brunel Assist portal and you will receive an email confirmation. Keep that email as it contains the ticket reference number.

  • The University will pass reports about club/society activity to the Union for our reference and follow-up where necessary.

  • Do not assume that the Union has seen your report, so please also email student.activities@brunel.ac.uk with a description of what happened and the Brunel Assist ticket number so that the Union can act faster if needed.


5) What to expect next

  • The University Health & Safety team will triage and investigate as appropriate and will contact the person who submitted the report if they need more information.

  • The Student Activities team will liaise with the University and may contact the club/society about follow-up, support or corrective actions.
